Services are offered by appointment only, please schedule online or call 225-456-7577. Prior to arriving please download, print and complete all patient intake forms. If you do not have a printer, we will be happy to fax or send you forms via the postal system. Completed forms will allow more time for massage therapy. Initial appointments include intake and review of all medical information.
Clinic Policies:
• Please present all coupons and gift certificates at the time of appointment.!
• We will be happy to accommodate late arrivals, however appointments will end at the
scheduled time and full charge will apply.
• $45.00 will be charged to your credit card on file for all appointments canceled or
missed without 24 hour prior notice.
• $35.00 charge for NSF checks and debit cards
• We follow HIPPA guidelines and will not share or discuss any of your treatment
without written consent.
• We accept cash, debit cards, checks, Visa, MasterCard, Discover and American
Express.
New Clients
Book any 60 minute massage for first time clients and receive $10 off. Please indicate in the notes section which type of massage you would like. We look forward to serving you.
